With the first factor, it is possible to roughly determine this yourself from home, although an examination from your surgeon will be needed to properly recommend the procedure or not. In general, if your nipples are below the crease of the breast, then you will need some sort of a lifting procedure as the implant alone will be unlikely to achieve a satisfactory result. If you have minimal breast tissue then it is unlikely that you will need a lift at all. Loose skin can also be a factor, as a breast lift can help to reshape and tighten your skin.
Your expectations for your final shape and your preferred position of your nipples will also play a part in whether or not you need a breast lift.
